Our philosophy of care is holistic and all-inclusive—providing physical, mental/emotional and social services through our Day Health Center. As a PACE program*, Senior Care Partners uses Medicare and Medicaid funding to serve members at our Day Health Centers free of charge. We also welcome adults who are not on Medicaid and Medicare for an affordable monthly cost. In addition to relaxing and engaging activities and savory meals created fresh by our culinary team, Senior Care Partners members benefit from an interdisciplinary team who integrate the care our members receive, according to a personalized weekly schedule. Senior Care Partners full-time physicians and therapy team are fully equipped and available to serve you. At Senior Care Partners, our community exists for you. Services Senior Care Partners P.A.C.E customizes its services around your specific needs. Through our Day Health Center, you can build friendships, enjoy your hobbies and still receive all necessary care and therapy. The standard services we provide are covered for individuals supported by Medicare and Medicaid. While many Senior Care Partners members come to the Day Center for social connection and recreational enjoyment, we have physical and occupational therapists as well as a team of medical specialists to design a holistic care plan around your needs. Our goal is to release any anxiety in your day. If for any reason you require medical assistance, we will be here to promptly respond with compassion and expertise. Our care includes all Medicare- and Medicaid-covered services. We can also provide additional hospital care and nursing home care. Eligibility All Senior Care Partners members must meet a basic set of criteria: 55 years of age or older Live in Calhoun County (all zip codes), Kalamazoo County (all zip codes), Eastern Van Buren County (zip codes: 49009, 49010, 49026, 49055, 49065, 49067, 49071, 49078, 49079, and 49087), Southern Allegan County (zip codes: 49070, 49078, 49080), or Southern Barry County (zip codes: 49012, 49017, 49021, 49046, 49050, 49060). Cleared to live safely as a member of this community and meet nursing home Level of Care Determination set forth by the Michigan Medicaid Nursing Facility Payment Senior Care Partners is covered monthly by Medicare and Medicaid. Most members pay nothing for our services. Please note, you are still liable to pay for any care you receive outside of our network. If you do not receive Medicare and Medicaid, you are still eligible to participate in our community. You can contact us for more information on cost or set up a tour of our Day Health Center.
